Steps for success
Faiths House is open to all children ages 3 months through 6 years of age. We provide hours for children attending full-time and before and after school.
There is a three-day minimum for all students enrolled in our programs. The application is then processed and a packet containing: a child information record, tuition contract, forms, and permission to publish form is initiated and sent to families via email. Families may request aprinted copy. When the office receives all completed paperwork, the child is enrolled, includingthe health appraisal and immunization record.
The following will be held in each child’s file.
• Enrollment Application with non-refundable registration fee
• Health Appraisal and Immunization Record
• Child Information Record (Emergency Card)
• Tuition Contract
• Immunizations or Waivers Form
• Permission to Publish Form
Before your child can begin attendance in the program the required health appraisal and current immunization record and any documentation specific to the child, including but not limited to, custody documents, allergy and asthma information, and any special needs instructions must be on file.
Faiths House will maintain and conduct all practices relating to enrollment, discipline, and all other terms and benefits of childcare services provided in a manner that does not discriminate against any child, parent, or family based on race, color, religion national, origin, sex, or handicap.
Recruitment is ongoing, and enrollment applications are accepted year-round.
At Faith’s House, we are committed to ensuring that all families stay informed about their child’s immunizations and physicals. Keeping vaccination records up to date is crucial for preventing illnesses and providing immunity against potentially life-threatening diseases. Vaccines are rigorously tested for safety and effectiveness and are recommended at specific ages to protect children.
To help families stay on track, we conduct regular checks to determine when each child needs updates. Notifications will be sent via email, individual texts, and memos to remind you of upcoming vaccinations. Please note that if a child's shot records are not current by the expiration date, a fee of $10 per week will apply until the records are updated.
We also provide educational resources, including brochures and handouts, and share information about local health clinics to support families in keeping their children healthy. For additional guidance, you can refer to the CDC’s Immunization Schedule Resources for Parents.
